Whatever you like or whatever your employer needs or go for CCIE.

What kind of work are you doing? What are you interested in? How do you want to progress your career?

You could do more of the same with CCIE R+S.
You could broaden your Cisco skills by taking another stream
You could get certified on another vendor like Juniper, Dell or HP
You could get some softer skills in project management, service management or security
You could get skills in Microsoft Server or Linux
You could get skills in cloud
You could get skills in virtualisation
    You could do a degree

If you really don't know, then the easiest thing might be to do the CCDA and CCDP exams. They are just one exam each if you already have CCNA/CCNP R+S, and are good if you are doing any network design/architect/engineering

I agree with OctalDump. Go for the CCDA/CCDP next. That's what I'm doing after CCNP R&S.

Do collaboration, wireless or security next. Then ccda and ccdp.
