SSCP Endorsement form - Previous Company Supervisors Name/Email ID

Hello Everybody,

I am in the process of filling up my SSCP endorsement form. I have been associated with the present company from 2009.
The endorsement form asks for previous employers supervisors name and email id. I kind of have lost all contacts with my last company supervisor and also they would not be in the same company till now.

Need advise on if i can leave the email id blank..

I've had the same problem with government employment applications. The State of Texas prefers applications over resumes. I just list my previous supervisors' names and the general company/agency phone number. You could also add a note alongside your endorsement form that states that some contacts are no longer valid.
