How to I submit the CISM Endorsement

Hi,

I passed my CISM and my boss has signed the endorsement form(CISM Application form).

I just paid the USD 50 for processing fee.

How should I proceed to send the CISM Endorsement form to ISACA?

Please advise. Thank you.
