Please House, i need your help.

I am apply for CISA Application. How do I fill the form? Am i to print it, fill with my pen, and scan back. ( in case i am sending by email).

I just want to be sure the form is to be printed because I couldn't type into the form.

They do have a web form that you can fill out, which generates the form you want in a printed style.

If your PDF software doesn't let you fill in that version, you can write your information in by pen and scan/e-mail it. Normally I would suggest you print in all capitol letters.
