Queston about Hotspots

This may seem like a silly question, but how do "Hotspot" questions work ?, do u have to place a marker on a specific place in an exhibit or on a specific answer in a question ?

If you go to the CCNA Prep Center there is an exam tutorial that has examples of all the questions types.
