disappearing image [update]

Hi guys
I have a problem with my 515 that i just got, for some reason after copying
the image from ftp and then rebooting the image in the flash becomes 0 bytes?? has anyone encountered such a thing? and of course i keep getting thrown into monitor mode. If you look closely flash is showing an 8mb pix722.bin i reload once and its still there. Now after a third reload...you can see that it can no longer find the image, I then load the image from tftp to ram.
and a dir flash: show taht the image is now 0 bytes. does anyone know what might be causing this? bad flash? bad hw? any input is much appreciated....
