Creating a Survey in Outlook

My boss has these questions that she wants us to send out to our company and we have no idea how to do it.

Has anyone created these type of questionnaires or surveys? Ideally I would like it to send us there response.

We did one to determine what all employees might want in the new snack machine we were getting. I ended up just doing a free survey at SurveyMonkey: Free online survey software & questionnaire tool. Worked great for us! Apparently the people here are really fond of Coke, Cheetos and Snickers.

The VP that we report to wants us to send one out asking people about the IT service and ticket response time.

Im not sure if the Outlook survey is the best tool to ask a lot of questions as it only allows you to answer a single question with answers given to the users, sort of a multiple choice question.
Good for voting where to go for lunch...

Sharepoint is good to make surveys with also.

Do you have a ticketing system with the capabilities to send out an email from a trigger once an incident is closed?

Couldn't you use the voting buttons and customize your message options?
