Authorization and Disclosure Form

Guys, I had my interview about 3 weeks ago and they called me two days later to fill out a form online for background check. Four days later, I called to see whats going on and the HR guy told me my background check went okay without any problem.
Now, the HR guy wants me to fill out an Authorization and Disclosure Form again. Why do they want me to do it again if they already got what they needed?

    Are you positive it is the same document? I would've asked him right there when he mentioned it.

    They asked me to fill out an online form with a background check company, which I did. Last week, the HR guy told me that my Background check went okay that they are waiting on my references , Now they want me to sign this form. I already sent him the form today. Should I call and ask?

    The authorization and disclosure form might be for your references/former employers, it gives them permission to ask questions beyond basic information. It is kinda like a HIPPA form in medical field. We use them here at my office.

    I'll just wait and hear from them sometime this week. I know they had problem contacting my references because they both left the company like 5 months ago. They probably need this signed documents to ask other people questions.

    Guys, I got the Job as a Desktop Specialist. Thank you guys for all your help and support. I'll be partying all weekend and can't wait to sign the offer letter tomorrow in person.
