Educate me on setting up SLA on a 3560

So, I was enlightened to the SLA way of monitoring network latency.
Site is set up that the 3560 is Layer 2, hand-off to an ISP router for Layer 3.
I have the PCs IP and port number, the destination and the service being used.
What else is needed and how do I set it up.
