Salary History - how to format it?

I have a question about salary history. I'm applying for a job and they request that your salary history be given. I'm just not sure how and what to list on it? My senario is this:

I had 2 non-paying internships in college.
I currently have only the one job that I've had since graduating college.
I currently do volunteer work on the side.

How and what do I list on my salary history. My full-time job is the only "salary\paying" job that I'd have to list.

Salary history should be kept off resumes and applications. Your salary information should be confidential. Potential employers ask for this information to ween out the high salary seekers and chose the ones in the low end.

The job is listed on careerbuilder.com and at the end of the job ad it's listd as is "please submit salary history, cover letter, and resume"

So should I just send my resume and cover letter?

I would but also I would state that my salary history is confidential and salary expectation is negotiable.
