Custom Microsoft Outlook 2010 form not displaying properly

Hopefully I can get some help with this. I've been googling but haven't found an answer yet. I created a custom form within MS Outlook 2010. I chose the 'Design A Form...' option and used the message format. Now, I just have a list of items and check marks that indicate a pass or fail. To start, I deleted the message window because I couldn't add checkboxes to the message area. Then I was able to create checkboxes, labels, and a note area. I published the form and opened it through Outlook. It looks great. I can add notes and use the checkboxes. To test, I filled out the form and sent it to myself. I get the email, but it is blank. I can't see it from the preview pane and I can't see it when I open the item. It looks like a blank email. Can anyone give me some ideas of what to try?
