Endorsement Question

When filling out the endorsement paperwork it asks for the examination #. Is this the [FONT=&quot]same as my certification ID #?

    My endorser completed the paperwork and submitted it via email and copied me. Upon review I see he did not check the box regarding "...suspected, charged, indicted, or convicted" of any crime. Not sure why he didn't ask me, as that would have fulfilled the "to the best of my knowledge" part. Has anyone here not had that box checked on their endorsement form? If so, were things delayed, audit triggered, etc?
