In my opinion, it is too wordy and unnecessary long for your experience.
Operating Systems:
All Microsoft Windows Operating Systems including 7, Vista, XP, 2000, 95, Server 2008, and Server 2012.
- Why list 95 and 2000?
Software:
Microsoft Office 2013 Suite and previous versions including Word, Excel, and Outlook, Active Directory, Microsoft Exchange Server, TCP/IP, Domain Controller, adding security policies to local, site, Domain and OU’s, backing up files into an external storage device, VMware virtualization knowledge and basic knowledge of Adobe Photoshop.
- Just saying Office suite is enough.
- What version of Exchange?
- Domain controller is not a software.
- Backing up files is a task.
- Although, TCP/IP is technically a software, it is generally not listed under Software but rather Networking. By putting it on the resume, you are giving me a permission to ask you a question about any protocol in the TCP/IP stack. Just be specific, for example: SMTP, FTP and etc.
Equipment:
Establishing and maintaining new PCs, workstations, printers and related equipment into workgroups or Domains. Setting up mobile devices such as Iphones, Andriod devices, and Blackberries.
- again, mixing equipment list with tasks.
- what's the difference between PC and a workstation? Is a workstation not a PC?
- i would completely remove phones.
Hardware:
Setting up networking equipment, such as basic knowledge of Cisco routers, switches, hubs, Network-Attached Storage (NAS), building PC towers and servers from scratch within budgets and deadlines.
- again, this is a list. what you did with it should be in the experience section.
- why is PC towers and servers separated from just PC's and workstations?
- isn't equipment and hardware the same thing?
- What is basic knowledge? Can you configure static routes and NAT? Or do you mean home routers? If latter is the case, I wouldn't even put it on the resume.
- Cisco hubs and NAS?
I would redo your experience with more technical details, instead of listing something like this:
Report all issues out of my scope to SBUMC Director of Information Technology Implementations.
At the end, you are trying to sell yourself so list the things which you think will impress your potential employer. Do you think I will be impressed that you helped somebody over the phone to burn CD or create an office document and hire you as a sys admin? Space on the resume is precious, use it wisely.

A PC and a WS are two different devices but I understand the point you are trying to make. Else I agree with the above, The layout isn't bad, but you could summarize Software, Equipment, and Hardware into a single heading. Give an overview then elaborate at the interview. Having a long list will likely bore the reader before they get to the work experience.

A lot of your examples could be shortened to make things more concise, such as:
Replace or repair damaged hardware parts, such as Motherboards, CPUs, Memory, Hard Drives, and related hardware. (two lines)
to
Upgrade or diagnose and repair all internal PC components (one line, taken from mine)
and you might want to state how big the networks you usually work on are, like this:
"Setting up Local Area Networks (LAN) with routers, access points, and hubs, while crimping CAT5e cables; securing wireless network with WPA2, Static Address, and securing router firmware with credentials and configuring router within the client’s needs."
