I worked with/on/around an AS400 recently that had not only hard-coded passwords but also hard coded workstation names and represented IP address withe 3 octets (it combined the last two into one, for example: 10.0.0.1 = 10.0.01).

More on topic, I see these in OLD equipment or home-brew/DIY devices that were built by someone that was lucky to complete the project (let alone secure it).
