When I worked at the Midway Museum, our cabling was horrendous. We had CAT3 running everywhere, including in the telephone room, in addition to the ship's original cabling, intercom system, etc. ~250 phone extensions, most were not labeled well, incorrectly, and had extremely tight cross connects. When I would go to trace a cable with my tone and probe, there were so many cables that the signal would jump. Not to mention I've seen almost every cabling standard broken while working there. I posted a couple of pictures on reddit.com/r/cablefail a few years back but they were removed.
