CISM Endorsement

Hi,

For CISM Endorsement, do I need to have a CV or just submit the this form? How many people need to endorse cos I heard two people ?

One thing to check, if you passed your CISSP, you need to submit endorsement within 9 months, for CISM, is it within 5 years ? Please help to confirm my doubt and any idea why ISACA is giving 5 years.
