Charging customers for Cisco voice maintenance

Hey guys & gals,
I'm just curious to find out (if any of you work in a managed services provider) what's your company's model for charging customers for maintenance.
Do you charge based on the good-ol' "price-per-port-per-month" or some other model (ex. per used license etc)?
