Salesforce Certs + CRM Upgrades

Hello All,
Our current license/support for Meditech Software which runs our HR and Payroll and Accounting services is going to expire by the end of next year. So we are shopping around for replacements, my boss said that the pricing for new software doesn't matter because Meditech is one of the more expensive EHR programs out there. I just want to know what is involved with upgrading a CRM. I'll be the administrator/manager of this project and need to know if our old system is compatible with any of the new CRMs out there like Salesforce.
Also if anybody has had experience in upgrading these systems please feel free to explain all that was involved. Specifically if you upgraded from meditech or a similar unix based system to a modern day cloud based system. I am doing this in the hopes that I could get some experience on salesforce and maybe squeeze out some certs or two from the non-profit where I work.
