Retrospectives are a good way to look at what worked, what didn't, what was learned, etc....
This is vendor biased but I really like some of the groupings.

It's important to collect Lessons Learned notes during the project's run rather than trying to look back in hindsight and remember what happened after it's all over and done. Having a separate "Lessons Learned" section in a project notebook or documentation system is helpful for jotting down things to remember later for creating the LL.
