A Practical Question About Yearly Performance Reviews/Evaluations

Hi guys,
I wanted to check with those of you that got the grade of "Consistently Exceeds Expectations" during your annual review. Can you share some of the tasks/projects you did that was pointed out during your review to make your manager decide to place you in this category. Please be candid and honest. I do know that their are sites where these criteria are detailed however since we're all in IT roles here I wanted to get practical examples. For reference I've shared the categories of the Performance Evaluation levels below.
