Another questions concerning ODOM book, possible errata!

Hello me again asking another mundane question.
Page 406 in Odom's ICND1 book goes in depth about router physical installation. Im confused about a certain step in the process that is written out. Although the process is clear and apparent; the way it is written is confusing, once again, to myself. My question is about Step 3 not Step 2. Here they both are in order.
Step 2 - If using an external CSU/DSU, connect the router's serial interface to the CSU/DSU, and the CSU/DSU to the link from the telco.
Step 3 - If using an internal CSU/DSU, connect the router's serial interface to the line from the telco.
Shouldn't the last part of step three read "connect the link from the telco to the CSU/DSU" since it is internal. Wouldn't the pressencse of an internal CSU/DSU eliminate the need for a serial cable in the first place?
Also, how would you connect a serial interface to a line from the telco? Is there some type of adapter? Aren't the connectors for four wire telco lines generally RJ-48?
