Upgrading/migrating from Windows NT domains...

Hi all,
I found in some practice questions stating:
"The xxx corporate wants to migrate its network" or "The xxx corporate wants to upgrade its current infrastructure from Windows NT 4 and Exchange Server 5.5 to Windows Server 2003 and Exchange Server 2003 ..." in a manner that seems to be almost interchangeable the term upgrade with migration.
Because I have no experience on Windows NT 4, I know only it's not AD and of course on migrations from that, I have problems understanding exactly if it is speaking about a scenario with:
Two forests and two Exchange Organizations
One forest and two Exchange Organizations
Etc.
Ther'is a way or a term-trick to single out what it is speaking?
I would love to not deploy NT domains only for this purpose.
