Building audio splitter box

One of my users has a need to split the surround sound audio they now have in their office to an earphone jack AND have the surround continue to play while being able to control the volume on the earphone jack. This is for a new device being marketed for those people with hearing aids.

Since this will go into one, maybe two offices, I figure its a DIY project. I did find a schematic for hooking up two devices (Wii and Playstation) to the same surround system, but I need to modify that so it will work as mentioned above. Anyone have an idea on how to split the audio from the three channels so that they come over to the earphone jack? I know I can put a potentiometer inline to adjust the volume for that jack, Im just not sure how to split the rest of the audio so it works all at once. Ideas anyone?

    If they're okay with stereo then you can just buy a plug splitter or make one by wiring two sockets in parallel and connect them to a plug. Impedance will be wrong but it'll work fine.
