Problem with reference. Problem applying for job. Advice?

1) Recently completed a contract job. Manager promised to write a reference letter mid-Dec. I emailed back if it's ok to have him as a reference for phone calls also (instead of just a reference letter). Ignored my e-mail. It's been about a month and I didn't get the letter. I have a feeling that the manager doesn't want me to move forward. So, if potential employers ask for a reference about this job, I have none! I'm in the process of applying to jobs and without a reference for this job...I'm in trouble and I'm lacking a bit of confidence. Advice?
2) I saw a job posting in my school database. It required me to send my transcript. I have decided to send an unofficial transcript (as it is very costly to get one printed. and if i dont get hired, it will be wasted). The problem is that my unofficial transcript in PDF format consist of my BIRTH DATE. and I cannot hide or remove it. Advice? Or should i just send it to the employer hoping that they won't realize that tiny detail on the transcript?
Many thanks

He might have had good intentions of getting something for you, but ended up side tracked and forgotten. I would call (not email) to figure out if he will still provide a letter or reference.MCTS: Windows Vista, Configuration

Thanks for your advice.
-I sent 1st email requesting a reference
-He replied and promised
-I sent 2nd email requesting phone reference as well
-No reply for weeks
-Reference letter not received
I will call soon in a few weeks to check it out. This is important to me because it's almost like my first real job I ever had (which relates to my field too). -

Why are you worried about the age on your transcript? I wouldn't worry about it personally. An expert is a man who has made all the mistakes which can be made.
