Create A Membership Form

Hello,

I am looking to create a membership form for customers who will be interested in joining my side health and beauty business. I know all the details that I want to add on the form but I am not really sure what program or application to use to achieve this?

Any ideas?

    An online form or a paper based form?

    If the former - other questions surface such as security, volumetrics and how you might want to further process the information arise.


    If a paper based form - then MS Word is as good a tool as you will need in my opinion.
