Microsoft Terms: Upgrade and Clean Install

I get a little confused sometimes how MS uses "upgrade." Sometimes it seems to indicate a true Upgrade (upgrading an existing installation) but sometimes it is used as simply doing a wipe and load clean install. For example here:
Common Migration Scenarios
It refers to a "PC refresh" (performing a clean install) as an upgrade. I took a practice exam, and it did the same, the process it described was a PC with Vista. The scenario described "upgrading the Vista PC to Windows 7." However the answers were referring to a wipe and load using USMT.
So...is "upgrade" sometimes used interchangeably? I guess I'll just have to use the answers to determine which meaning it is using.
