Now we all get those plaques when we receive our initial certification. Now do we get a new plaque when we renew or that first one is it. I say this because the plaques say the certification expires on X date...

Answer from SANS - You will receive an updated Certificate in the mail showing the new expiration date once your certification is renewed.

Just a paper certificate and not an new plaque? Maybe that'd be another $19. I can see that.

its another $19 for shipping and handling for the new plaque. but you can get a new one with updated info.
