autocad 2014 help.

Hey guys need some help. Im am currently in an autocad class and my professor wants us to submit our work via usb and wants the file to basically look like an 8x11 peice of paper with a square display at the top for the draft and with the name assignment and dat below. She did show us how to do this very quickly but jve forgotten and havent been able to find it in my book or onljne yet. Help appreciated!!!

it's years since I've touched AutoCAD so I dunno if it's any help..

You need to create an A size format. Haven't touched autocad since the late 90's, so you'll need to figure out how to create a format.
