Indexing Guidance

So I am sitting for my second SANS exam at the end of next week. I took and passed MGT 512 without any index or tabbing of my books, just winged it, and scored a 90, but it was a stressful exam and I had almost no time left at the end of the exam. I was thinking of trying the indexing technique many use, but am really confused on where to start. Does anyone have any suggestions or advice on how to create an index? Thanks!
